has officially changed its registered business name to InLife Benefits Insurance Company, Inc.
Members who are still under Generali will continue to be accepted and accommodated at our clinic. Rest assured that your existing coverage remains valid during this transition.
At the same time, members of InLife Benefits Insurance may now begin to avail of our Basic Physical Therapy services, subject to the requirements listed below.
Requirements for Approval of Claims
To ensure smooth processing, kindly prepare the following:
- Full Name of the Member
- One (1) Valid ID
- HMO Card (must be valid/not expired at the time of approval)
- Date of Birth
- Diagnosis from an Orthopedic or Rehabilitation Doctor
(Please note: we do not yet have an accredited Generali HMO doctor.) - Referring Doctor
- Active Mobile Number and Email Address
