Requirements for Claims with HPPI Members
- 1Complete Full Name
- Active HPPI Card Member ID
- Company Name and Date of Birth
- 1 Valid ID
- Diagnosis/Presciption/ Referral from an Ortho or Rehab Doctor that is accredited by HPPI since we do not have an in-house rehab doctor for consultation yet.
- Chief Complaint and Pertinent Findings
